Diabetes Self Management Program

Diabetes affects approximately 16 million people in the United States.  If left untreated, diabetes can result in serious complications, including heart disease, blindness, kidney failure, nerve damage and amputation.

In addition, Program clinicians often refer patients who are experiencing problems with circulation or vascular concerns as a complication of their diabetes to vascular and endovascular surgeons on staff at Quincy Medical Center.

The Diabetes Self Management Education Program has created a Diabetes Survival Skills Worksheet to assist patients in managing their diabetes prior to be seeing in the Program.
